All student-athletes must complete a physical examination before participating in tryouts or practices. Download and complete this form with your physician.

• Must be signed by a licensed physician
• Valid for current school year only
• Submit to coaching staff before first practice
• Include emergency contact information
• Parent/guardian signature required
